What Does This Ultrasound Report For Abdominal Pain Indicate?
Under above circumstances I am confused what to do next. Please go through his USG report and suggest whether we can wait for another 10 days or not.
CT scan abdomen and pelvis with contrast is recommended to take decision
Detailed Answer:
Hi,
Thanks for writing in to us.
I have read through your query in detail.
Please find my observations below.
1. Your son is having features of acute appenidicitis on ultrasound. However the appearance of inflammed appendix is not mentioned in detail.
2. The features of leukocytosis and increased CRP might indicate acute appendicitis.
3. As an expert in ultrasound I recommend doing a contrast enhanced CT scan of abdomen and pelvis for your son. CT scan gives detailed information and will tell us if surgery is required immediately or csn wait for 10 days. Any complications of acute appendicitis will also be known. After doing CT scan the surgeon will get clear idea if surgery is required immediately or whether it can be treated with medicine and decision can be taken after exams.
Hope your query is answered.
Please do write back if you have any doubts.
Regards,
Will be doing CT Scan soon.
In the meantime am worried about the word you have used "ÄCUTE", does that mean it is very severe?.
I have come across who had appendicitis but they didnt immediately do the operation. Does that mean their cases were not acute?
I also read that if the pain suddenly disappears it means the appendicities has burst, is there any prior indication to that? like anything to show that it is going to burst.
Please adavice
Acute appendicitis is used for sudden onset inflammation of appendix
Detailed Answer:
Hi,
Thanks for writing back with an update.
1. It is good to know that you have decided to proceed with CT scan and hopefully we will get a much clear idea and whether operation is required.
2. Acute is a word used with signs of appendicitis which is of sudden onset. Your son was completely healthy before and since 4 days he is having pain Therefore word acute is used. Some people have mild pain and we use sub acute appendicitis for them.
3. There are people who have pain in appendix region on and off for months and in this case we tell it is acute on chronic appendicitis.
4. The severity of your sons condition cannot be accurately understood by the ultrasound. I mean there are many things like the size, whether there is any collection in the appendix area and if a mass or abscess has formed. In severe cases appendix can burst and form gangrene and this is emergency condition. To know the exact situation in your son, I have recommended a CT scan and detailed information will be known with respect to size, severity, any complication like rupture or mass formation.
5. The pain suddenly decreasing does not always mean a burst appendix. Patient can continue to have pain in abdomen after appendicitis has burst. An appendicitis that is going to burst will have a large size and show signs of acute inflammation. There are some features like visualization of appendicolith or stone inside the appendix which have a higher rate of rupture. A CT scan will tell these things and if there is any urgent requirement for surgery.
6. An uncomplicated acute appendicitis can be treated with medicines and this is why the people you came across did not require surgery immediately. This is also one of the reasons I insist on an early CT scan abdomen and pelvis with contrast for your son.
7. Please be in touch with the surgeon if the pain suddenly increases through the night.
Hope your query is answered.
Please do write back if you have any doubts.

Pantium 40
Zanocin-OZ
spasmolar
I read that spasmolar and Pantium 40 are for bowel movements and gas.
However, Zanocin-OZ appears to be Ofloxacin which is used to treat bacterial infections including upper and lower urinary tract infections, lower respiratory tract infections, skin and soft tissue infections and sexually transmitted infections (of urethra and cervix). I DONT SEE ANYTHING WRITTEN ABOUT APPENDITICITES and this medicine has wide range of side effects.
I am reluctant to give Zanocin-OZ to him because of above.
Please advice
You can give him zanocin OZ if he is 18 years of age or older
Detailed Answer:
Hi,
Thanks for writing back with an update.
1. Spasmolar is given to decrease the pain from appendicitis.
2. Pantium 40 is for preventing gastritis.
3. Zanocin OZ is antibiotic combination and has been used to treat acute appendicitis and there is research literature stating the same. It is alright to prescribe Zanocin OZ for patients of acute appendicitis who are more than 18 years of age. Below 18 years age Zanocin OZ can cause bone, joint and tendon problems and pain.
In conclusion, if your son is older than 18 years then it is safe to give Zanocin OZ. It he has not yet reached 18 years age then please go back to doctor and take antibiotic recommended for children below 18 years of age.
