Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Male/51 - dull aching pain in scrotum - for about a month - ultrasound shows veins on both sides- more on left side- wants to know whether the condition will improve without doing anything...
Please give some additional information:
First of all please post the full report of ultrasound and color Doppler to know the grade of varicocele so that proper opinion can be given.
Please mention if you have any other associated symptoms or diseases.
Married? If yes, kids?
Semen analysis if done.
Please answer all questions so that I can assist you better.

Bilateral small hydroceles are noted . Bilateral varicoceles are also noted . There are also bilateral reducible direct inguinal hernias , measuring on the right 2.1 cm and 2.5 cm on the left. I did go see a specialist regarding my hernias and it was determined it was not the cause of my symptoms, hope this helps thx
Brief Answer:
As discussed in details.
Detailed Answer:
Thanks for your feedback.
Varicocele on both sides appear to be small - can not be really the cause of dull ache. These too can be secondary to Epididymo-orchitis which you have mentioned got treated.
Bilateral hydrocele: Varicocele does not cause hydrocele. Small hydrocele are usually called secondary hydrocele and are due to infection of the epididymis and/or testes called Epididymo-orchitis. Since you have history of infection and this may be the cause of your pain.
Small hernia can cause pain only if of indirect variety; you have direct hernia and they usually do not cause pain.
Ultrasound may not catch infection in every case unless very marked.
Hence I would advise you the following:
Tests of blood.
Tests of urine: routine, microscopy, culture and sensitivity - and if there is an evidence of infection get antibiotic and anti-inflammatory medication till cure.
Also get an ultrasonography of the abdomen, particularly for genitourinary system- prostate in particular.
I hope this answers your query.
Give scrotal support.
Avoid hot and humid atmosphere.
As discussed in details.
