Microvascular gliosis refers to old lesions in the brain caused by decreased blood supply to brain in certain parts. These are small areas, which appear hyperintense (white) on T2W and FLAIR sequences of MRI brain. Gliosis is the scar tissue due to old ischemia (lack of blood flow to brain).

Old lacunar infarcts refers to small blood clots in the brain, which are caused by lack of blood flow to brain.

Both Microvascular gliosis and lacunar infarcts occur in people with certain risk factors such as high blood pressure, sugar or cholesterol.

Usually treatment is required to control the underlying risk factors, and aspirin is advised to prevent the occurrence of brain strokes in future.
