We do not, I'm told, have a qualified ear specialist here in XXXXXXX Is this a simple , common procedure or should it be entrusted only by a specialist? Does it ever seal off by time or by itself?

If it doesn't then it needs to be sealed off surgically. It requires microsurgery hence specialist intervention is must.
