Perilymph fistula is an abnormal connection in the thin membranes that separate the air filled middle ear and the fluid filled perilymphatic space of the inner ear. This small opening allows perilymph to enter the middle ear.
This leads to imbalance while walking.
It needs to be sealed off.

We do not, I'm told, have a qualified ear specialist here in XXXXXXX Is this a simple , common procedure or should it be entrusted only by a specialist? Does it ever seal off by time or by itself?

It might get sealed off over a period of time.
If it doesn't then it needs to be sealed off surgically. It requires microsurgery hence specialist intervention is must.
I advice you to wait for another six months to see if it gets healed on its own.
