Considering those metastatic lesions, it was obviously Dukes C colon cancer amd now this is advanced stage of malignant disease. Surgery in cases of multiple metastases has not any role and only hope in such cases is to start with chemotherapy which may kill lymph nodes metastases and then this one in liver can be surgically removed.

If chemo dont take effect,unfortunately, further treatment is limited. Ablation and other percutaneous intervention can be done to manage liver met but it is useless for lymph nodes metastases. in this case liver met is not worrying since it will grow slowly and progression of the disease will come form paraaortal lymph nodes. You should start with 2nd and 3 rd line of chemo or with experimental therapy if you can find some. 5 year survival in these cases is less than 5%. 1 year survival is approx. 50% , 2 years 25-30%. 3 and more can be expected in only cca 10% of people.

I am not sure that nano knife can kill paraaortal metastases. it can affect liver mets and it is used for it in most cases but if other 2 secondary lesions cant be completely destroyed,then there is no purpose of treating liver mets with nanoknife when these liver lesions can be treated by other more conventional methods: surgery, chemoembolisation,etanol injection or electrocoagulation.
If radiologist think that all metastases can be successfully treated by nanoknife then you can try with it but chemotherapy should be continued if you ask me

Yes, it definitely is especially if patient is in good health and have motivation for aggressive but efficient treatment. Nano knife radiation should be combined with chemotherapy. It may have significant systemic side effects but this is only way to achieve long term survival. At this point it is more important to see effect of nano knife on paraaortal lymph nodes and subpleural node and if there is good result then the same can be done for liver met or liver met can be surgically removed.

Experience with nano knives is limited but in some people there is very good response and if you have available nano knife then you should go for it.

yes, the main problem may be if tumor infiltrate duodenum,small intestines and cause obstruction. or if liver met infiltrate main billiary ducts and cause jaundice. it is hard to say when or will it happen,but usually tumors must be larger than 10 cm to cause it or it must be located near these important organs. In every case,if it cant be completely cured, then it definitely can be slowed down by chemo or radioth. Of course, complete cure isnt impossible and about 5% of people may be cured. The dynamics of mets growth is variable,but in most cases it doesnt occur suddenly and grow relatively slow. Only mets on small intestines and parietal peritoneum called carcinosis, show rapid progression. Liver and lympoh nodes mets are less progressive, but it would be best to repeat ct scan in 2-3 months an deverything will be clear. I really hope that treatment will show some good results.
