What Is The Cause And Treatment For Rash In Groin Area?
I had rash in my groin area in the past(8 yrs back) and I took treatment for that.After that the rash disappeared,but it left some discoloration(black) in my groin area.
I have consulted a local doctor and at first he suggested me to use keralin ointment (morning & night) along with clotrimazole powder.He also prescribed me flucanozole 150mg for the first 10 alternative days. It worked partially, but once I stopped taking flucanozole there was no improvement.
So, after 20 days he prescribed me candid-b lotion(daytime) and lobate-gm cream for night use along with one AF-400 mg. It worked well but very slowly. It took nearly 1 and half month and the discoloration was almost gone(95%). Suddenly one day it started itching and the skin turned reddish. After that within a week the area turned black once again. I am very much worried with this outcome.
One observation that I made during this period was that the ointment and lotion is working only when I continue taking flucanozole.
Would you please suggest me some medicine to treat this discoloration.

Thank you for writing to us on HCM.
Your observation regarding the darkness is very accurate.
The fact is you are suffering from fungal infection called Tinea cruris and it is the factor leading to darkness over groin folds. Since your fungal infection is not treated adequately, the darkness/pigmentation also doesnot resolve completely.
Thus I would guide you along the management in such a way that we completely treat fungal infection and so the associated darkness of groin.
I would recommend 15 days course of oral Itraconazole 100mg twice daily after meals.
Apply a cream containing antifungal agent like Luliconazole twice a day over affected sites.(LULIFIN cream)
Also use antifungal dusting powder on regular basis,especially when going outdoor.(ONABET dusting powder)
Maintain hygiene and use ketocoanzole soap for daily rinsing of affected areas.
Use loose undergarments like boxers instead of routine underwear and iron your boxers before putting it on.
I am sure you will be fine soon with the treatment highlighted.
Please make sure you update me your condition after 2 weeks with the follow up pictures so that I can guide you further along the management.

I have the following 2 doubts sir.Could you please resolve them.
1.Can I use any dusting powder such as candid etc. or should I use onabet. Onabet is sometimes hard to find here.
2.My first doctor asked me to sprinkle the dusting powder after applying the lotion and cream on the affected area. Should I repeat the same procedure or just apply the cream and leave it and only use powder when I go out.

1. Yes you can opt for other dusting powder like Candid but its better to use newer molecule like sertaconazole (ONABET) considering the growing resistance to older molecules like clotrimazole( CANDID). Anyways if there is availability issue its okay if you use Candid powder.
2. Yes I would advise the application of dusting powder in same way twice a day after application of antifungal cream but in addition also apply it everytime you are going out.

I have uploaded latest pictures. I have been using the medicines prescribed by you for the last 2 weeks. But sir, as you can see from the pics there has been slight to no improvement in terms of color. I think the texture of the skin has changed slightly.
Could you please tell me what to do next sir. I am getting troubled by this very much and getting demotivated to use any medicine.

The darkness of skin is called post-inflammatory hyperpigmentation. It is common after any inflammation or infection. The pigmentation is not very easy to treat and it takes months for normal skin to appear. As the reason for pigmentation is your fungal infection, it is thus very important that we cure your fungal infection completely. I would insist you continue the treatment I advised earlier for atleast one and half month by which time we can expect the infection to resolve completely.
Once your infection is resolved the darkness will subside on its own in few months.

Yes, take oral itraconazole for 15 more days and continue Lulifin cream ( change to lulifin if you are using any other cream !) for one month. Also continue dusting powder and antifungal soap. After one month please post me your pictures for further management.
