Hi and thank you so much for this query.

I have reviewed the image provided. This very much looks like impetigo which is a bacteria infection. If that is the only crusted lesion you have, then we do not need antibiotics for this now. Cleansing with an antiseptic like iodine would guide it towards healing. However, if there are some more than just this single lesion, I will suggest that you consult a doctor to get antibiotics as they can only be obtained with a doctors prescription. If you are in pains, Tylenol would be able to help.

In all, looks like a bacteria infection. Single lesions need just cleansing with an antiseptic and not antibiotics. Monitoring and cleansing would be enough unless something else comes up.

It is not a viral infection like herpes....i.e how do you know its bacterial and not viral?

Hi,

If you have had herpes before, and these sore are repetitive, then it could be herpes. But rarely would a primary herpes infection presents with just a single lesion. Singles lesions are more common with bacterial infections. Herpetic lesions would present as an ulcer with clean bases, I didn't see this on inspection.

Well, for now I will suggest that you mange as outlined above. If it recurs in future, then testing for herpes would make sense but would not want to with a single lesion and more so if it is the first time you are having this.
