Reports clearly show 2 important changes- Reduced GFR s/o CKD and Severe Vitamin D deficiency, which require immediate correction through oral vitamin D supplementation.
With regards to your further questions, here are my answers
1. Hypothyroidism is the causative factor for CKD in your case even though both are temporally related.
2. CKD is always a progressive condition despite aggressive measures like diet modification, low salt intake or control of Hypothyroidism.
For Vitamin D deficiency, I generally prescribe oral pills of vitamin D( cholecalciferol) 60 K IU, one pill once a week for 12 weeks. After which vitamin D levels should be reassessed to determine further course of action.
