What Is The Prognosis For Grade 1 Tubular Breast Cancer?

Question: I have been diagnosed with grade 1 tubular breast cancer in both breasts. First lump 0.4mm in left breast, was excised last November. Second lump, 0.8mm in right breast was excised in December. 2 lumph nodes from left clear, 3 lymph nodes from right clear so both node biopsies negative. Both tumours er positive. I do not have any details as to weather there was any lymphatic invasion or the % tubularity of tumours. I would like to know how tubular breast cancer is classified in the uk, I.e. Whether 90% tubularity is required for this diagnosis, a nuclear grade 1 and rare mitosis. The prognosis if this is the case is good i understand.

Hi

Tubular breast cancer is classified in the UK by the amount of gland formation which is how well the tumor cells try to recreate normal glands, the nuclear features which is how ugly the tumor cells look and the mitotic activity which is how much the tumor cells are dividing.

Each of these features is scored from 1-3, and then each score is added to give a final total score ranging from 3-9. The final total score is used to determine the grade.

Grade 1 tumors have a score of 3-5.

Grade 2 tumors have a score of 6-7.

Grade 3 tumors have a score of 8-9.

90 % Tubularity is required for the diagnosis of nuclear grade 1 tumours.

The prognosis is good in your case and treatment consist of surgery followed by radiotherapy.

Tubular cancers are almost always sensitive to hormonal therapy.

Hormonal therapy for a number of years after surgery is recommended to reduce the risk of the cancer coming back.

Grade 1 tubular breast cancer has an excellent prognosis and survival.

In grade 1 tubular breast cancer disease free survival at 5 years is 97% and 93% at 10 years.

In all research I have done, in strict criteria of 90% tubularity, no deaths occurred in long term studies and some studies also suggested it could be almost benign at 90% tubularity.mis this correct, my oncologist also stated she had never seen a recurrence or metastisis of it. What do you think?
Also how does bilateral breast carcinoma affect survival here, with twice recurrence risk?

Yes you are right with 90 % tubularity no death occured in long term studies and sometimes it is benign.

Metastasis is very rare in this type of carcinoma.

Since it is grade 1 tubular breast carcinoma bilateral breast cancer will not effect survival and prognosis is good.
