Brief Answer:
cardiac arrhtymia
Detailed Answer:
Dear sir,
Thank you for providing me the relevant clinical data.
Here are the possible causes for your condition:
1) Heart arrhythmia which causes a decreased cardiac output and syncopal episode. I would suggest a Holter monitoring to rule out paroxysmal arrhythmias.
2) Orthostatic hypotension
3) Benign paroxysmal positional vertigo
4) Situational syncope.
We can perform a Diller hall pike maneouver test to rule bening paroxysma positional vertigo. This is done by ENT doctor.
I hope this helps you out. If you have any more questions, please do not hesitate to write back to me.
Yours sincerely,
Dr Thakker.

THYROID LEVELS
Detailed Answer:
DEAR SIR,
Thank you for this information. Are you able to upload an ECG rhythm strip?
If your holter recording has shown episodes of fast heart rate we need to know the cause for the same. Atenolol is the right drug for a fast heart rate. You can have a thyroid level checked to make sure you don't have hyperthyroidism. That can cause paroxysmal tachycardia.
I hope this helps you out. If you have any more questions, please do not hesitate to write back to me.
Yours sincerely,
Dr Thakker.

Brief Answer:
MRI brain to rule out VBI
Detailed Answer:
Dear Sir,
Atenolol can cause orthostatic hypotension. However you have mentioned that your blood pressure has always been normal in all positions. Hence this rules out the cause. Thyroid report normal is reassuring.
I would suggest MRI brain to rule out a vertibro-basilar insufficiency. If the circulation to the posterior circulation of the brain is compromised, this can lead to a similar condition.
I hope this helps you out. If you have any more questions, please do not hesitate to write back to me.
Yours sincerely,
Dr Thakker.

Brief Answer:
Please read the response
Detailed Answer:
Dear sir,
For patients who are suffering from essential hypertension and not on treatment, their renal excretion mechanisms are not functioning well. Also the kidney has a limit to which it can deal with excess sodium and fluid load. If this is exceeded, it can lead to high BP. In your case you are on treatment and have good kidney function hence the brooth did not change your BP.
