today we did a ante natal scan doctor said single gestational is visible but no fetal pole and heart beat,sac measurement is 8w 3d

the scan report

the uterus is enlarged with a single gestational sac
fetal pole could not be seen
the GS measurment c/w 8w 3d of gestation
the sac is not regular and decidual reaction is weak
no obvious presac bleed seen
no adenexal pathology seen
internal os is closed

Hi,
Thanks for writing in to us.

There might be wrong dates as it is termed, when there may not be visualised fetal pole corresponding to the time since last menstrual period. This can lead to absent fetal pole and cardiac activity.

Another cause might be a blighted ovum. In this there is abnormality in fusion between sperm and egg and this leads to non viable pregnancy and only gestation sac is seen.

In both situations, a repeat ultrasound scan is necessary after a week to document any further development as expected in a normal pregnancy. Any diagnosis can be made only then.

Hello, I would be happy to help you with your question.

If you are supposed to be 8+ weeks by your last menstrual period, and the sac size is 8+ weeks, and there is no fetus within the sac, then this is consistent with a miscarriage, sometimes referred to as an anembryonic pregnancy. You need HCG levels drawn and consideration of a D&C to further evaluate and treat. This is clearly not a normal pregnancy.
