Dear sir

Let me pick you some of the general suggestions that we give to patients with the complaints of breathing problem and chest pain.

The common cause for chest pain in patient with breathing problems are
1. Increased fluid volume
2. Increased resistance in the blood vessels
3. Sluggish pulmonary circulation causing pulmonary edema
4. Autonomic neuropathy.
5. Decreased kidney function

As I reviewed your medications history, it is appearing good. But additionally
1. Control your blood pressure adequately with in 130/80 mm of hg
2. Avoid taking oral medicines for diabetes and start insulins if you have uncontrolled diabetes.
3. Develop a good exercise tolerance level by doing regular physical activity and exercise

Thanks
