Is this swelling present on the abdomen for the long time or it as occured recently is very important to know what is the cause for the swelling. As you are telling it has spread to back with out any itching, the swelling should be examined then only we can come to the diagnosis. Regarding having the ringworm infection on the shoulder can be controlled by applying some anti fungal cream over the lesion.If it is not coming down you may require oral antifungal drugs which should be taken once a week for 6-8 weeks. But these drugs can be taken only after getting the prescription from your doctor.
