Are Enlarged Spleen, Back And Ribs Pain Signs Of Leukemia?

Mother has MDS, receiving transfusions for 3 years now. About every 2 weeks at this point. Checking for enlarged spleen (back, rib pain). Could this signal a progression of the disease (leukemia)... And are we looking at end of life at this point? Very, very fatigued, & only 1 week out from previous 2 unit transfusion.
