OBGYN, Maternal and Fetal Medicine 's  Response
Hi, I have gone through your question. In my opinion, your wife is having a condition called intrahepatic cholestasis of pregnancy, which usually develops in the third trimester of pregnancy. In this condition, there is itching specially over palms and soles, and the ALT, AST in the liver function tests are raised.
ALP can sometimes be raised, but it is raised upto 2 times normally in pregnancy.
Since she also has the history of increased BP records, HELLP syndrome in which there is increased BP, hemolysis, elevated liver enzymes and low platelet count needs to be ruled out.
I would suggest you to consult your OBGYN specialist so that examination and appropriate investigations can be done, and prompt treatment can be done for favourable maternal and fetal benefit.
Hope this answers your question.
Wishing you both good health.
