Hi,
It seems that you might have Bellano-prosthitis giving this problem.
Due to not cleaning of smegma properly under fore skin can be resulted adherent fore skin to glans penis resulting unable to avert fore skin or rolling up fore skin from glans.
You may require circumcision operation.
Consult surgeon and get examined.
Ok and take care.

Psychiatrist Dr. Anjana Rao Kavoor's  Response
Hi,
Thanks for writing in to us.
Balanitis is infection of the foreskin, for which you are already taking treatment. Some times due to severity of infection, the foreskin might get adherent to the glans penis and it gets difficult to retract it. Under such situations, never try to forcefully pull back the foreskin.
You must report to your doctor and take his guidance. He will try to pull back in a gentle way and as much possible. If it does not pull back and the urethral opening is obscured, the adherent foreskin may be released through circumcision surgery.

Hope this helps.
