I had gone through your query and understand your concern.
First i have to what is smudge cells and the other possible causes.
Smudge cells are actually lymphocytes.
They're artifacts produced by the rupture of fragile cells during the process of the blood smear preparation.
They're associated with both malignant and
reactive lymphocytosis, but a high number is more often seen in chronic
lymphocytic leukemia.
They're also common in
infectious mononucleosis, pertussis and lymphomas involving peripheral blood. They're rarely encountered in the normal blood smear.
Thus number of smudge cells per 100 lymphocytes is also important in CLL.
To confirm the CLL, the following features shoulb be considered.
1. Lymphocytosis must persist for longer than 3 months.
2. Peripheral blood flow cytometry is the most valuable test to confirm a diagnosis of chronic lymphocytic leukemia (chronic lymphoid leukemia, CLL).

Ultrasound abdomen may show
splenomegaly.
4.
Lymph node biopsy may be necessary to confirm the diagnosis.
Your Flow cytometry results are negative.
Thus consider the other possible causes for smudge cells ( Infectious mononucleosis and other lymphomas )
How ever smudge may also present in normal blood smear.
Finally if nothing is found in investigation, consider lymph node and
bone marrow biopsy examination.
