Hi,I am Dr. Amitkumar Sharma (Internal Medicine Specialist). I will be looking into your question and guiding you through the process. Please write your question below.
my was diagnosed with bicuspid aortic valve about 2 years ago. he is followed by a cardiologist who started him on lisinopril, gradually increasing the dose to 15 mg daily. he recently told me he was having severe night sweats with no other symptoms. we have a scheduled apt with his D. in three weeks. do you see any reason to go in earlier.
Thank you for your query. I can understand your concerns. Approximately 1/3rd Bicuspid aortic valve becomes stenotic. ACE inhibitors like lisinopril should be avoided in moderate or severe aortic stenosis. ACE inhibitors should be used with caution but are beneficial in treating patients with symptomatic LV systolic dysfunction who are not candidates for surgery. They should be initiated at low doses and increased slowly to target doses, avoiding hypotension. Lisinopril induced hypotension can cause sweating. With fixed valvular obstruction to out-flow, vaso-dilatation (ACE inhibitors) reduces pressure distal to obstruction without increasing cardiac output.
