when Methotrexate is not keeping the RA in control and the doctor wants to add Enbrel , knowing the serious side effects (like cancer) is this a good choice ? person had thyroid cancer (had surgery ) in high school and now is 42 years old . Hard to make this decision not knowing the percent of risk involved with all the serious side effects possible.
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.
To be honest anti TNF, are only recommended in RA if the disease is not controlled with two of drugs used for at least six months. There is no recommendation for its use if the disease is controlled with conventional drugs. There are many side-effects with these anti TNF, however the previous debate of it's increased risk of malignancy is now settled and its said that it does not increase the risk of malignancy. Regarding its use with methorexate, ( it these are indicated ) yes, it should be used with methotexate as it has been shown to increase the efficacy and if methtexate is not contraindicated.
Hope I have answered your query. Let me know if I can assist you further.
