General & Family Physician 's  Response
Chronic dry eye can cause blurred vision, contact lens intolerance, increased risk of infection. Appropriate diagnosis and management is important. Dry eye can be classified into two categories - evaporative and tear deficient dry eye.
Meibomium gland dysfunction is the prevalent cause of evaporative dry eye causing an unstable tear film and increased evaporation. Meibomitis is another cause. Evaporative dry eye is worsened with exposure to irritants including air conditioning, dry heat, low humidity, wind and intense concentration associated with close work and computer use.
Aqueous tear deficiency is more prevalent in females possibly associated with menopausal factors. Common causes of tear deficient dry eye include Sjogren’s syndrome, diabetes, contact lens wear and some medicines.
Advice
1.     Stop using Nepalact eye drop
2.     Tear plus eye drop – one drop in each eye four times daily
3.     Betamethasone eye drop (Betnesol eye drop) or dexamethasone eye drop (Lasone eye drop) – one drop in each two time daily for 15 days
4.     Hot fomentation with slight warm water three times daily
5.     Keep your eyelid clean
6. Revital capsule - one capsule daily for 1 month to improve nutritional deficiency 7. Get your eyelid examination done by an experienced eye specialist who might also recommend test for tear film instability
Thanks
