I have a family member that has Parkinson disease and has hallucinations. He has been prescribed Nuplazid 17 mg. He is also taking sotalol 40 mg BID. There is a precaution with Nuplazid and Sotalol. How serious is the combination. We have not started the nuplazid at this time.

Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

It is not safe to take Nuplazid while on Sotalol drug. Nuplazid is prescribed for the treatment of hallucinations and delusions associated with Parkinson disease psychosis.

When taken along with Sotalol, it can increase the risk of "Torsades de pointes" (an irregular heart rhythm) that may be serious and potentially life-threatening.

Your family member may be more susceptible if he has a heart condition called congenital long QT syndrome, other cardiac diseases, conduction abnormalities, or electrolyte disturbances (for example, magnesium or potassium loss due to severe or prolonged diarrhea or vomiting).

Your family member should seek immediate medical attention if he develops sudden dizziness, lightheadedness, fainting, shortness of breath, or palpitations during treatment with these medications, whether together or alone.

He may be prescribed an alternate effective drug or adjust the dose of the prescribed medications.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Mohammed Taher Ali
