Can A Falling Face Down Cause Headache, Unsteadiness And Dizziness?

Hi,



I had a bad fall four weeks ago, tripped and slammed into flagstone paving face down, right on nose. Since then I've had some headaches, occasional pain behind right eye, wooziness, and unsteadiness. My doctor doesn't think I should be worried. Should I be checked again?

Hi, thanks for asking from HCM.


I can understand your concern. As you are still having symptoms of headache, pain right eye, dizziness, it might be due to

- Concussion injury - Concussion means brief loss of consciousness (LOC) with no visible brain injury. It can cause headache, irritability, vertigo for few weeks. Resolution of all symptoms is the rule. To suspect it, you must have normal CT head and history of LOC.

- Fracture of frontal or right orbital wall - It can be easily missed if no imaging was done at the time of injury. It can cause persistent headache, vertigo for 1-2 months. It ca be ruled out by NCCT head.

- Chronic subdural hematoma - It always present late. It occurs due to gradual accumulation of blood. It can cause above symptoms. Confirmation is by getting NCCT head. Large hematoma needs evacuation while small hematoma can be managed by fluids and medication.

- Right eye disease like refractive error/Glaucoma/soft tissue injury/hematoma It can be confirmed after consulting ophthalmologist. He will examine you and check your visual acuity, intraoccqular pressure to make diagnosis.

The good thing in your case, is that already 4 weeks have passed and you are conscious, oriented. It means your injury was mild. But you should consult neurosurgeon first and get CT head done, if your symptoms are worsening and not responding to medication. For some relief, the symptoms can be managed by

- Painkillers like diclofenac or paracetamol after proper prescription.

- dizziness, unsteadiness can be managed by betahistine after proper prescription.

- Avoid strenuous activities and repeated head injury.
