definitely both can be placed. actually new generation pacemaker has both the function. pacemaker and defibrillation are two different work and can be done by new pacemaker.
pacemaker works when our heart rate slow down or stops, while defibrillaotr works when our heart beats in a rapid rate of more than 350 /minutes.

they do have a pacemaker combined with a defibrillater that can be implanted, the only thing is how old is the person, what is there quality of life going to be if an internal device keeps there heart going, if the person is up and about and active, it is good, as a nurse, i have seen several with trachs and on vents because the pacer/defiber keeps thier heart beating, and that is sad
