Hi and welcome to HCM. A dull pain in arm and shoulder may indicate angina (symptoms of heart attack), but not always. Your age, any other risk factors like high BP, Diabetes, High cholesterol, family history of heart attack all should be taken in to consideration to assess your risk to heart attack. Big toe pain is not a indicator of heart problem, if the pain is more intense in night time it may indicate gout. Your doctor has done right thing to diagnose this. Blood tests showing hyperuricemia can support the diagnosis. For pain in left arm and shoulder your doctor after complete examination may refer to orthopedician. Treating gout first priority is to relieve pain and shorten the acute attack. NSAIDs (nonsteroidal anti-inflammatory drugs) such as indomethacin, diclofenac, ketoprofen, and naproxen are the mainstay of treatment. All the best, Regards
