Can Enlarged Labia Lips Be Due To Sexual Abuse?

My aunt is in a nursing home, alzheimer, unable to get out of bed, has trach, mental retardation, no longer talks. For a few months she has had enlarged labia lips. She also has a catheter for past few years. Should I be concerned about sexual abuse? They did test but found no STDs or infections.

I don't think it is due to sexual abuse
I think it is happening from muscular relax and long use of urinar catheter
