Hi,

Fecal lactoferrin is a marker of inflammation in the intestinal tract, and a positive result may suggest the presence of inflammatory bowel disease (IBD), such as ulcerative colitis or Crohn's disease. However, other conditions such as infections, ischemic colitis, and medication-induced diarrhea can also result in elevated fecal lactoferrin levels.

Your symptoms of constant diarrhea, occasional blood in the stool, and improvement with fiber intake suggest that you may be suffering from IBD. Neomycin is an antibiotic that can help alleviate symptoms by reducing the number of harmful bacteria in the gut. Lomotil, on the other hand, is an antidiarrheal medication that can slow down gut motility and may not be effective in treating the underlying inflammation.

It's important to follow up with your doctor to discuss your symptoms and test results. Your doctor may recommend additional testing or refer you to a gastroenterologist for further evaluation and treatment. Depending on the underlying cause of your symptoms, treatment options may include anti-inflammatory medication, immunosuppressive therapy, or surgery. In the meantime, you can continue to manage your symptoms with a high-fiber diet, hydration, and over-the-counter medications like anti-diarrheals or pain relievers as advised by your doctor.

Take care. Hope I have answered your question. Let me know if I can assist you further.
