Hello and welcome to HCM,
Recurrence of pain can be due to reappearance of anal fissure.
Bumps in the surrounding region can be due to anal tag, anal wart or just an inflammatory process.
However, anal fissure is not directly associated with formation of bumps and itching in surrounding region.
You can continue with sitz bath.
But at the same time you need to consult your surgeon for clinical assessment.
If there is recurrence of fissures, sitz baths have to be continued along with stool softeners.
If the bumps formed around the anal fissures appear to be due to warts or tags, surgical excision may be required.
Thus, consult your surgeon for further management.

Thanks and take care
