Diabetes Type 2. Elevated Sugar Level. Suggested Intake Of Amaryl Instead Of Januvia And Amaryl. Is This Ok? Which Is Better Between Metlong And Glycomet To Control Sugar?
Dear Doctor,
I am diabetic with type 2DM for last 14 years. Was taking Amaryl 3 mg and Plita 15 mg before meal. Recently, my sugar level has elevated and I was suggested to take:
- januvia 100 mg and amaryl 2 mg before meal
- metformin 1 mg twice a day.
My family physician suggests to take only amaryl 3 mg before meal instead of taking both januvia and amaryl.
Therefore, I would like to know what is the difference between januvia and amaryl and whether it would be advisable to take only Amaryl.
Secondly, what is the difference between Metlong and Glycomet SR? Which would be good to control blood sugar?

You're a Type II Diabetic for the last 14 years and OHA's(Sulfonylureas and Thioglitazones). Both the dose are more than Half dose of the maximum. There are long term complications of the drugs which should be kept in mind. Pioglitazone is associated with increased risk of bladder carcinoma. Januvia is Sitagliptin. It is a DPP-4 inhibitor. It scores over pioglitazone.
Metlong and Glycomet Sr are the Sustained release preparations of Metformin. Advantages are Single dose and sustained levels of metformin in the blood to effectively control blood sugar by insulin sensitisation.
