DEAR SIR/MADAM

PIOGLITAZONE AND METFORMIN, BOTH ARE INSULIN SENSITIZERS, ITS ACTIONS ARE VERY LIMITED ON FASTING BLOOD SUGARS.

SECONDLY PIOGLITAZONE IS ASSOCIATED WITH FRACTURES AND BLADDER CANCER, SO IT IS BANNED BY INDIAN GOVERNMENT RECENTLY 2 DAYS BACK. SO YOU MIGHT NOT GET THESE MEDICINES OVER THE COUNTER.

IF YOU WANT TO CONTROL FASTING VALUES THEN ADD ONE SECRETOGOGUES SUCH AS GLIMEPERIDE 1 MG AT NIGHT TIMES, YOUR FASTING SUGARS WILL COME DOWN. BUT WATCH OUT FOR HYPOGLYCEMIA.

THANKS

Internal Medicine Specialist Dr. Arindam Ray's  Response
you can take metformin(walaphage) upto 2 gm /day.....recently there is big controversy on pioglitazone(?banned),so preferably stop this drug....low dose long acting insulin is other option, like glargine..starting dose can be 10-12 u s/c at 7 pm....it better controls ur fasting blood sugar, without risk of hypoglycemia....check ur serum creatinine,HbA1C....

General & Family Physician Dr. Pavan Kumar Gupta's  Response
Hello
You must control your sugar meticulously.
It is not possible for a doctor online to treat you since it requires various parameters and constant monitoring.
You must consult your doctor regularly and take his help in controlling not only diabetes but also any other associated complications,if any.
I hope you understand this.
Thanks
