Diagnosis T1N3 Stage 3b Adenocarcinoma Of Lung, Primary Left Lower

Diagnosis T1N3 stage 3b adenocarcinoma of lung, primary left lower lobe. Has had chemo and radio therapy.
Most recent CT scan report states - new pulmonary nodules in the right lung with post radiation changes including collapse of the medial aspect of the left upper and lower lobes. Interval increase in size of left perihilar mass with multiple new nodular pulmonary densities on the right suggestive of metastatic disease. Sclerotic bone lesion at the C5, C7 and T1 levels not seen on previous imaging, likely new metastatic change.
The patient has severe COPD. Can you break down what this report is stating and what it means?
