Do I Need A Full Hysterectomy For Cysts In Ovaries And Enlarged Uterus,and Should I Be Worried About Cancer?

Hi. I've just been told I have 2 complex cysts 7.5 cm on right ovary and 3.5 on left plus a 10!cm enlarged uterus. I'm 47 and the doctor believes I also have endometriosis and has recommended a full hysterectomy ASAP. Do I have to be worried about cancer and in your opinion is this surgery the roué to go

Hello and welcome,

I understand your concern completely.

The cysts are big and you have already said that they are complex. Your age is also 47. For any woman after 45 with complex cysts and enlarged uterus if childbearing is completed then we advise a hysterectomy.

The reasons are in general the chances of recurrence of these is high and they are in both ovaries so if both ovaries are removed then in any case the hormonal support is lost. So keeping the uterus is not logical as uterus does not produce the hormones. So keeping it means just keeping a vestigeal organ and leaving the window for any cancers to develop in it in future. So this is not rational. If the ovaries are removed the uterus is better removed too. In your case, if endometriosis is suspected then surgery itself may be difficult. Again it becomes more and more difficult to do repeated surgeries as scarring from previous surgeries changes the anatomy as well as tissue quality. Your cysts are big and they need a surgery and an examination under a microscope for a final diagnosis and future treatment.

About the risk of cancer, you age is in the medium range where the risk is not very high. But considering the complex and bilateral nature of cysts [ in both ovaries], the possibility must be kept as one of the possible diagnosis. Certain investigations like CA 125, LDH, AFP, HCG help to determine the possibility of cancer and can be done after a consultation with your doctor.

These are the reasons that the doctor advised a complete hysterectomy in your case. Please discuss all this with your doctor and I am always here if there are any more doubts.
