Does Discolored Sinus Drainage Need ER Attention?

This is concerning my mother: 72 yo w female, hx of mini strokes, was on med. At the time that was linked to this as a side effect, on blood-thinner, just dx hypothyroidism, started 50mcg synthroid 3days ago, woke this morning with slight headache, small blood clot in mouth which she spit out, noticed sinus drainage is tinged bright pink (from sinus, not chest.) Instructed by her doctor via phone consult this morning this is probably sinus related, not stroke activity and is in process of scheduling her with ENT today. Does this sound reasonable to you or should we be considering ER and MRI?

Hi,

Noted the history related to your Mother having had mini stroke, started with Synthroid and after few days developed red tinged nasal discharge and blood clot in mouth in the morning.
As told by your Doctor the problem may be related to one of the following:
Nasal and sinus infection.
Bleeding disorder.
Side effect of Synthroid.
Hence I would advise you to take her to ENT if possible so that a proper clinical evaluation, examination, ENT Endoscopy can tell us the cause supplemented by CT scan of sinuses and tests of blood for coagulopathy or infection and get a proper diagnosis.
Till then instill decongestant nasal drops.
No harsh cleaning of nose.
Continue antibiotic if she is already on.
Once a proper diagnosis is done then it is possible to have proper evidence based medicines and get well.
Continue medicines for stroke and any other medical diseases as possible.
Also see that she might have been prescribed blood thinners for stroke and may need to hold after discussing with your Doctors.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, ENT and General Surgeon
