Hi I am Dr mohamed ibrahim resident of physical medicine and rheumatology , I am a researcher my thesis is in comparing severity of neuropathy in patients on Long term insulin verses oral antidiabetic drugs

All patients examined on early long term insulin therapy have Less severe neuropathy than others on antidiabetic drugs

Does early agressive insulin initiation in type 2 Diabetic patients decreases the neural insulin resistance on the long term ?

I would love to answer this question, all the patients those are having type 2 diabetes does not show sign of neuropathy even with the history of taking oral hypoglycaemic drugs with the high level of glucose levels in all the three status, with the poor drug compliance, on other hand some patients have complain of neuropathy those are taking insulin, in either form or type with control diabetes status and good drug compliance, this indicates that the oral diabetes agents or insulin is not the matter as long as the neuropathy is concern, but it could be something to take with the genetic constitution of individuals, some kind of micro mutation would be responsible and that is more common in male than female after the 5th decade, and this is not just the coincidence, neural insulin resistance is likely if certain gene destructed, (this topic needs long discussion, this is just brief I gave it here and hope may help you ) HCM will really appreciate your feedback, thank again have nice day.
