Bleeding at any stage of
pregnancy is not a good sign. It may indicate separation of placenta causing bleeding.
At 34 weeks, spotting could be due to different causes:
1. You may be about to start the premature labour. This is the first diagnosis that strikes me. Low back ache is suggesting that the mouth of the uterus is having strain on it and is trying to open.
2. The placenta could be low lying and little bit of it is separated
Placenta praevia). Usually such bleeding is painless, causeless and recurrent. First episode usually very minor. However the subsequent episodes are quite alarming and profuse.
3. Normally situated placenta might start separating (abruptio placentae). In such case there is risk to the baby. However, this type of bleeding is usually associated with uterine contractions and pain.
The diagnosis and differential diagnosis of second and third condition is done by
ultrasonography.
4. Any local lesion on the mouth of the uterus.
You should report to your Obstetrician ASAP. If it is threatening premature labour, the you will be given steroid to help the maturation of baby's lungs. Moreover, some uterine relaxants may be tried to revert the labour procedure. You will be admitted in hospital. You should be in a facility where Neonatal Intensive Care Unit is available.
If it is placenta praevia, you will be admitted in hospital for the rest of pregnancy. Any alarming bleeding will be dealt with
caesarean section. In placenta praevia,
vaginal delivery cannot be attempted as there is a risk of excessive bleeding. The placenta is in front of the baby.
3. I feel, the chances of abruptio placenta are very remote since you do not have pain.
4. Cervical lesions will be located only on internal examination and can be treated accordingly.
Please do not delay. Report to the Obstetrician. Till that, restrict yourself to the complete, bedpan
bed rest. Do not latch the rest room if at all you go to. There is no necessity to have bath.

medical help, your pregnancy may go ahead uneventfully.
