Does Swelling Of The Esophagus Wall Cause Progressive Dysphagia?

Hi Dr , I have a 34 year old nephew complaining of progressive dysphagia since 6 months back. Drs told him it was due to swelling of the esophagus wall. What is the actual diagnosis or condition. What is the management of this case? I am a retired nurse. Thank you.

Welcome at Healthcare Magic. I read your question and I understand your concern.
Progressive dysphagia has many causes.
1.Achalasia
2.Diffuse esophageal spasm
3. Functional dysphagia
4. Eosinophilic esophagitis
5. Carcinoma
If it's only for solid and there is no history of anemia or weight loss then carcinoma is less likely.
If it is both for solids and liquids but no systemic symptoms like weight loss then there is high probability of achalsia.
if there is history of allergy then eosinophilic esophagitis will be on the top of differential diagnosis.
He needs barium swallow and upper GI endoscopy and then proceed accordingly.
Treatment is different in each case. In Achalasia treatment options are nitrates, calcium channel blockers, injection botulinum toxin and esophagomyotomy. In case of Diffuse esophageal spasm (common causes are acid reflux and achalasia) cause should be treated. In case of Carcinoma surgery is the treatment of choice. While in case of eosinophilic esophagitis steriods and avoidance of allergens is required.
As doctor has told you that there is swelling then it correlates with esophagitis.
