Hello,
Welcome to Healthcaremagic.
From your query i understood that you have uncontrolled diabetes.

But chest pain, syncope and episodes of pass out are all related to heart disease.

You may have some heart disease like Myocardial infarction, Valvular heart disease like Aortic stenosis, Arrhythmia or Heart block etc

It is better to take an ECG, Echocardiogram immediately.

And also you need to check your Blood pressure and pulse rate by a doctor.
If all these are normal, Do upper GI endoscopy and take proton pump inhibitors like Omeprazole.

Recheck your HbA1C , Fasting and Postprandial blood sugar, Hemoglobin.

Hope this helps.
Take care.

General & Family Physician Dr. Pavan Kumar Gupta's  Response
Hello,
Feeling sleepy clearly indicates uncontrolled diabetes. There would definitely be deranged blood pressure levels along with dyslipidemia as these are very commonly found in diabetics than in non diabetics.
your symptoms of heaviness in chest and pain in arms clearly suggest presence of coronary artery disease and it demands a cardiac evaluation by a cardiologist immediately and urgently.
Prevalence of coronary artery disease is much higher in diabetics as compared to non diabetics. You must make sure that you keep your blood sugar, blood pressure and serum cholesterol levels in tight range.
Thanks
