Episodes Of Vertigo While Getting Up, Lying Down. BP, Sugar Normal. Relief With Verticare, Symptoms Persist

i am male 55 yrs. weight 78kg.
i am having transient episodes of vertigo since 10 days while getting up from bed or while lying down which last for a few seconds.
my blood sugar is normal, blood pressure is 130/90.
there is no cervical pain, no nausea vomiting, no hearing difficulty.
i was prescribed verticare (betahistine 16mg) 3 times a day for 5 days.
i am having some relief but the symptoms has not resolved completely

Hi,
From history it seems that you might be having postural hypotension.
In this condition there is sudden fall of blood pressure from lying down position and sudden stand up position.
Go for blood pressure checking in lying down position and standing up position.
There is another possibility of having some middle ear infection or otosclerosis of small ear bones.

Vertigo can be caused by central or peripheral causes. Central causes mean that it is related to the brain or spinal cord.

Peripheral causes are more common than central causes.

The most common peripheral causes are related to the inner ear and include conditions such as BPPV, meniere's disease and vestibular neuronitis.

Based on the fact that you stated your episodes are intermittent and related to your movement, it is possible that it is caused by BPPV (benign paroxysmal positional vertigo).

In addition to the medication that you are using, it would be helpful to do the epley or semont maneuver. These maneuvers have been shown to be very helpful in this condition.
Your doctor can help you perform one of them.

hello and welcome to HCM,

At the offset let me assure you that what you are feeling is due to a condition called the benign positional paroxysmal vertigo or BPPV which is one of the most common and benign causes of vertigo.

In this condition, there are small stones or rocks as you called it which are called the otoliths which gets disturbed from their place and migrate to another place and cause the giddiness.

Although it sounds very complicated, it's a very common cause of giddiness and in most cases it is a self limiting condition.

All you need to do is wait for the stones to either dissolve or get back to it's original place.

The treatment involves the use of medication and certain guidelines which you must follow. You may consider these:



1. If your symptoms are not fully controlled then you can add another anti vertigo tablet with cinnarazine 75 mg in the noon. To make sure that it does not clash with your betahistine medication, you can take betahistine in dose of 24mg twice. Your doctor will know about these

2. You need to take betahistine 24 mg twice or 48 mg once or 16 mg thrice for another minimum two weeks in consultation with your doctor.

3. During this time which can last for around two weeks to a month, you need to do everything very slowly and avoid sudden moments and bending down or looking suddenly up. When you get up or look up or down, you must do it very very slowly and gradually. Basically you must avoid any sudden movement.

4.While climbing stairs always hold the side railing..




If it persists then you need to do consult an ENT and he may suggest an exercise called the epleys which you can easily google and if you need the steps i can help you with that.

Meanwhile do not strain and do not worry.

As you are having vertigo in both getting up from bed & also lying down, it is a positional vertigo. Not Postural Hypotension which only causes vertigo in getting up position. As it is transient, the most probable diagnosis is Benign Peroxysmal Positional Vertigo or BPPV. In BPPV, hearing is not affected but the balance system within the ear is affected. Please complete the course of Verticare & then go see an ENT specialist who treats vertigo.
