Fever, Calpol Given. Done With Blood Test. Can Any On Line Doctor Help Me To Read This?
Hello doctor, my daughter had fever (i.e varied temperatures with a gap of 4 to 5 hours), calpol was given to her, after 1 hour of calpol dosage the fever used to come down completely. This scenario continued for 6days and 7th day our doctor advised for blood tests, the report says that IgM Antibodies to S.typhi (later flow negative Immunoassay), IgG antibodies to S. typhi (Lateral Flow Negative Immunoassay). WIDAL test: Salmonella typhi "O" Antigen: Positive upto 1 in 320 serum dilutions and Salmonella typhi "H" antigen : Positive upto 1 in 320 serum dilutions.Salmonella paratyphi "AH" Antigen and "BH" Antigen negative: KINDLY NEED YOUR SUGGESTIONS / ADVISE.
Hi! welcome to HCM! your daughter had fever for 7 days which came down with calpol but again showed a rise.calpol is paracetamol which is an antipyretic that just brings down fever and gives a symptomatic relief but cause of fever persists.
Blood tests showed negative IgM and IgG negative but positive widal test .A. positive Widal test is based on a fourfold rise in O agglutinins in repeated tests or a titre of > 1:160 (in endemic areas like India) in a single test.Since fever is persisting and titre is 1 in 320 so it probably is typhoid. Regarding negative antibodies to S.typhi (later flow negative Immunoassay), IgG antibodies to S. typhi (Lateral Flow Negative Immunoassay) there can be false negatives. My advice is to get a culture done as the culture isolation of Salmonella typhi from blood and bone marrow is the standard diagnostic test to confirm typhoid fever.
