Fever, Lethargic And Cranky. Tested For Leukemia But Negative. Anything To Concern?

Hello , my 3 year old son gets a fever almost every month on the dot , it runs at about 38-39c and lasts about 3-5 days. He has no other symptoms except that he is a bit lethargic and cranky , this has been going on almost a year , I have seen doctors who have tested him for primary complex and leukemia , all negative , what else could cause this ?

Is there any weight loss?
Does he have any chills or rigors associated with this fever?
Is there any contact with a case of tuberculosis?
