For The Last 6 Years Or So, I Have Noticed

For the last 6 years or so, I have noticed that anytime I laugh hard, cough, or tilt my head back to stretch I would get pains in my head that are best described as the pain you feel when you get a brain freeze. I would have to stop whatever was causing it and relax to make it go away, if I did this several times within a short period of time, I would end up getting a headache from it. Lately I have noticed I now am having near syncope when I do these things now, laugh hard, cough hard, or tilt my head back for an extended period. I have had cat scan performed when it first started and it was normal. Nothing noted. The neurologist diagnosed me with migraines, although I had no other symptoms or a migraine. With it now causing near syncope it is a lot more scarier for me. What can cause or bring this on? Is there a certain thing I should have my family doctor look for? Thank you for your time.
