Gum Over Lower Wisdom Tooth On Left Side Swollen, Paining, Pain Stopped After Pushing It Back . Causes ?

I fell asleep earlier today and when I woke up a few hours earlier a part of my gum over my lower wisdom tooth of the left side was swollen and in a lot of pain. Well it was kind of going over the other tooth as well so I pushed it back. When I did that, the swelling went down and nothing but blood came out of it. What could cause.this to happen?

hello and welcome to HCM forum,
i am glad you wrote in, first of all, i would like to inform you that the gum tissue that is covering your third molar or the wisdom tooth is infected which in turn has resulted in a swelling of the same, it is called "pericoronitis", it is a very common condition which occurs prior to eruption of third molars.
i would advise you to start with warm saline rinses(3-4 times/day) followed by, antibacterial(betadine) mouth rinses(twice/day) for a week. as the gum or the soft tissue flap is swollen it sometimes bleed if excessive pressure is applied over it, do not worry kindly visit your dentist , he/she might prescribe antibiotics and anti-inflammatory medication.
i hope i answered your query,
i wish you good health,
take care.

hello,
have your wisdom tooth got erupted or is it still covered by a flap?
AS per the description given by you,it is still impacted and the flap overlying the tooth has infected which is known as pericoronitis.there is food lodgement under the gingival flap.
it exhibits chronic inflammation for a long period.
if the debris and bacteria are deeply entrapped an abscess may form.it is a mixed infection and various bacteria of dental plaque play a significant role.
pain and swelling of pericoronal tissue around the affected tooth, difficulty in chewing, difficulty in opening mouth are the usual complaints.mildly ill with fever, malaise and regional lymphadenopathy ,throat and ear infection are the associated symptoms.
entrapped food debris must be removed.
a radiograph help in assessing the position of involved tooth.if horizonally impacted or if there is inadequate space ,the tooth must be removed.if it is in a favourable position, surgical removal of pericoronal flap should be done.
administration of antibiotics helps to relieve the symptoms and prevent spread of infection to adjacent tissue spaces.
regards,
Dr.Bindiya.

It is pericoronitis. The gum over the wisdom tooth is inflamed.
You can start with tinimox tab twice daily for 5 days and Flozen AA tab twice daily after food and Pantaprozole 40mg before food to avoid gastric irritation and can apply dentogel over affected area twice or thrice daily and maintain the area clean and use some luke warm water and salt in it rinse thoroughly.
If it is still persisting, it might require small surgery called operculectomy i.e excision of the gums covering the tooth. this is done if the wisdom is properly erupted without troubling the adjacent tissues otherwise wisdom tooth needs to be extracted.

Thanks & Regards,

OBGYN Dr. Dr. Asra Ishtiaq Ahmed's  Response
Hi Dear,
You are suffering from a common problem called Pericoronitis and Your lower wisdom tooth is probably partially or fully impacted (this will be more evident by clinical and radio graphical assessment)
Pericoronitis is an acute infection in which there is swelling of the gums which may be a result of poor oral hygiene or any sort of mechanical trauma ( during chewing)
You should start with warm saline rinses 4-5 times a day and antibiotic therapy. (Amoxycillin- 500 mg twice daily for 5 days)
the swelling will subside. But I would suggest you visit your dentist soon and get the wisdom tooth extracted as this problem may recur when there is food lodgement or trauma to the tooth and surrounding tissues.
I hope this helps you

Take care
Thanks

Dentist Dr. Neha Sumra's  Response
Hello
Welcome to HCM
I went through your history & it suggests that you are suffering from Pericoronitis i.e. inflammation of pericoronal tissue before eruption of third molar. It occurs due to food lodgement & bacterial accumulation in that area. Its Symptoms are:-
-pain
-swelling
-difficulty in mouth opening
-fever
-malaise
-regional lymphadenopathy
Get an IOPAR done & consult your dentist. If flap is interrupting in its path then go for flap removal i.e. operculumectomy. Otherwise if sufficient space is not available then go for extraction.
For temporarily relief do warm saline rinses 3-4 times a day.
Start with Tab Amclaid-625 twice a day along with Tab Zerodol -SP thrice a day for 5 days.In Case of gastric problem take rabeprazole-40mg once daily.
Do betadine gargles twice a day.

Take Care

Dentist Dr. Geoffrey Ward's  Response
Hello and thank you for your question.
What you have is tissue over growth called an operculum. This when get inflamed leads to pericoronitis. You pushed it back so it was no longer getting bit on and being irritated by the teeth. You either need to have the dentist or oral surgeon cut it off or extract the tooth to remove the issue. The tooth isnt' fully erupted so the tissue remains partly over the tooth and allows for debris to get caught under it.

Best Regards

Dr. Ward
