Had Ear Pain, Swelling In Ankles, Pain, Tender. Related To Tonsillitis?

my daughter is 8.5 yrs old she had some ear pain 2 months ago then the dr said it was referred from tonsills and so we did some blood test there was elevated ASOT level 520 the Dr prescribed penicillin G 1.2 IU one injection every 3 weeks.
she took 2 shots till now but then suddenly yesterday she got swelling in both ankles the swelling is mainly in the front of the foot and is painful and tender limiting the movement and today she had another swelling in one knee and swelling in the fingers and she complained of pain in the wrist too.
are these swellings related to the tonsillitis and what test should we do now to properly diagnose??

Welcome in H.C.M.

I have co-related all facts of your clinical history with the investigation report (ASO titre) and the ear pain to your child having tonsillitis.

I am suspecting that pain in the ankles are due to rheumatic arthritis because ASO titre of your child is raised, ear ache is again may be due to extension of infection from throat to middle ear via eustachian tube. This story needs investigations to know the real facts . thus you should do-

1- Culture and sensitivity of throat swab.

2- Gram's staining of throat swab.

3- Thorough examination of heart to rule out any valvular heart disease.

4- Blood- CBC, ESR, serum creatinin, SGPT, SGOT, C-Reative proteins.

After this child should be thoroughly examined by physician to rule out any complications of strep infection and finally with all due precautions tonsillectomy should be planned to avoid further complications.
