Hello & welcome,

If you have a black hole on back right tooth, that clearly indicates development of caries on the particular tooth.

As top or extreme upper part of the same tooth is hurting you, that means the tooth is somewhat badly carious or there is broken tooth structure due to caries.

Such teeth usually have sharp edges or walls which tends to hurt to cheeks or tongue specially.

If it's hurting seriously, then you can take any mild painkiller till you make visit to your dentist which is very important here. This is because your dentist will grind those sharp edges or walls of the tooth followed by restoration of the same tooth & this will definitely solve your problem unless the caries is progressed to the pulp or innermost part of the tooth.

If caries have reached the pulp, then only the treatment will be changed & dentist may go for Root Canal Treatment which will be decided after taking diagnostic radiograph.

Take Care.

hi
the hole in your tooth is a carious lesion that requires root canal therapy...

in case you don't have time to visit your dentist, you can place clove oil dressing for time being and can take ibuprofen also... you can take it three times a day...

but you should get your RCT done and should take proper medication (antibiotics and analgesics) as recommended by your dentist...

Brush and floss your teeth daily.

Hi
The black hole is indicative of caries and pain indicates that your tooth is deeply carious.
Treatment for this is root canal treatment.
Consult a dentist, follow antibiotic prescription.
Right now you can take Ibuprofen.
Take care
