Have Full Thickness Tear Of Supraspinatus With Retraction. Pain In Arm. History Of Bicep Tenotomy. Recommendation?

What would you recommend for a full thickness tear of the supraspinatus with a 2 cm retraction? I'm currently 3 months post op from a massive tear right next to where this new one is located (It required 5 stitches and an anchor inside). I'm a musician and a music conductor so my shoulder is really needed.
There is currently pain all the way down my upper arm, following the bicep and brachialis to right above the elbow. I has a bicep tenotomy about 3 years ago because it ruptured. Scar tissue actually reattached it so quickly it never fell into my arm. I'm 47.
We (surgeon, PT, and myself) are all scratching our heads as to how this new tear occurred. I'm trying to weigh my options.

Hi, you had a tear in the shoulder, previously , besides that there is another tear, it may be due to the contrature of the 1st tear. any how it is also should be repaired.
The orthopedic treatments should have rest as a main treatment. You would not have given proper rest after the surgeries .
I prescribe neurotropic injections for 10 consecutive days, besides pain medication to my patients.
I advise you to take more of green leafy vegetables,pulses,sprouts, and protein richfoods for muscle strength.
