I understand your problem.
If the odor is so bad and periods are irregular, it most likely indicates an infection or possibly also a cancer.
When patients with similar complaints walk into my clinic, the first step is a complete clinical and vaginal examination, cervical and vaginal swabs for culture and microscopic examination, a
pap smear and an
ultrasound.
The next step is a cervical and
endometrial biopsy [best is a fractional curettage procedure or a biopsy using
hysteroscopy where one can directly see the endometrium]. I also send samples from the endometrium for culture to detect any infection. The procedure is best performed just before you expect a period as it yields maximum possible sample for diagnosis.
Both these procedures would help arriving at the cause and further treatment would depend upon the cause.
