Have Pericoronitis With Mild Trismis. Cheek Is Swollen. Have Radiating Pain In Head, Ear. Took Penicillin. Time To Cure?

I have pericoronitis that started yesterday with mild trismis. Today, the entire left cheek is swollen as well as lymph nodes. I have radiating pain to my head, ear, eyes. I had a routine dental xray on Tuesday for referral to Oral Surgeon. Dentist called in penicillin for me and I've already taken 2,000mg today (per recommended dose). I'm using saline rinses every 2 hours or so and taking ibuprofen. What else can I do for relief? How long should I expect these symptoms to continue? I can't talk or open my mouth enough to brush.

Hello,

Thanks for choosing HCM forum.

Pericoronitis is due the impacted wisdom tooth which is erupting.

Depending on the space available for tooth eruption,either flap overlying tooth has to be removed to enhance tooth eruption or tooth has to be extracted.

This can relieve pain.

Meanwhile,gargle with lukewarm saline.

Take medicines complete course.

Thanks for sharing your concern.

you are already taking the necessary and appropriate treatment required for pericoronitis.Here i would like to add a small procedure called saline irrigation.

It is done by isotonic saline solution and a bent needle.Adequate amount of saline is taken in the syringe and applied over the affected area.

This is done four to five times.This is very effective.your dentist can teach you once and then you can do it on our own at home.

Also ,do not worry the symptoms will subside.but please remember that, if required go for third molar extraction,rather than suffering again,as it is recurrent in nature.

Welcome to HCM forum,
As you said that you Have pericoronitis, cheek swelling and mild trismus.
consult doctor for cleaning the area below operculum(flap covering wisdom tooth) with betadine and normal saline.
Afterwards place a thin cotton pellet soaked in local anaesthetic agent under the operculum after flushing it.
It will give you belief.
Follow antibiotic prescription and Take FLANIL tablet for swelling.
Maintain good oral hygiene.
Do warm saline rinses after every meal and use antiseptic mouthwash.
Apply hot fermentation to reduce trismus.

thanks for the query...
well, for symptomatic relief, please visit your dentist and request him to irrigate the area properly with saline or any antiseptic solution...
he can also make the swollen area out of occlusion if any tooth cusp is irritating it by grinding the tooth cusp slightly...
also go for anti inflammatory drug along with antibiotics...
warm saline rinses are must...
as your X-ray has already been taken so it will show the space available for your tooth to erupt...
if sufficient space is available, then go for a simple surgical procedure called operculectomy in which dentist will remove the flap covering your wisdom tooth...
is sufficient space is no available then go for its extraction...

hello friend,
pericoronitis is a term given to the inflamed gingival over the partially erupted tooth,most commonly in 3rd molars(wisdom teeth);as you have already taken antibiotics hence its not recommended to go for another one immediately.therefore i would suggest to go for a final step that is either excision of the coronal flap(which is less invasive) or if its the 3rd molar which is considered as a vestigial portion get it extracted(more invasive procedure)...

Thanks for asking the query,

Difficulty in mouth opening is due to pain and swelling.
I would encourage you to go for the extraction of your wisdom tooth.
Trismus and pain in head and ear is due to dental origin.

Take the complete course of antibiotics and analgesics prescribed to you.
At home take lukewarm saline rinses and antiseptic mouthwash gargles.
Avoid eating from the affected side.
Avoid eating of hot and spicy foodstuffs.
Maintain a good oral hygiene.

hi there,
my dear friend its a great pleasure to help you here. As you say you are suffering from pericoronitis. i think the treatment line you have been given is correct. only if you don't get relieved what can be done is a process called chemical cauterisation or electric cauterisation.it will remove the unwanted tissue flap covering your third molar.this tissue flap is called pericoronal flap and its inflammation is called pericoronitis. when this inflammed tissue is removed your problem will be solved.

Hello,

Thank you for writing at HCM,

Pericoronitis generally occurs due to infection caused predomonantly by anaerobic bacteria...as you have already started antibiotic and warm saline rinses..its good...

But you have to consider why its happening.. go for proper X-ray of that region...within 5-7 days it will be cured...You can apply metrogyl gel on that region,it will give quick relief...

After comfirmation with radiograph if extraction is suggested then go for it...or operculectomy also can be done.....

Without this you may get temporary relief but recurrence can occur later on...

thanks for the query...
well... it seems to be due to erupting third molar...
food accumulation between the gum and tooth often causes inflammation...

you should visit your dentist and should get an IOPA done... request him to irrigate and clean the area between your tooth and gum... he will also make the inflamed area out of occlusion by grinding the opposing tooth if required... antibiotics and anti inflammatory drugs are required too... do warm saline rinses 6-7 times a day...

another thing is to check the space for your tooth to erupt.. if space seems to be available on the radiograph, then go for a small procedure called operculectomy... in which inflamed tissue over the tooth is removed...
if space is not available, then please go for its extraction...
