Have Protruding, Slightly Knotty And Tender Vein In Arm. Skin Very Dry And Crinkly. Guide

I have a protruding, slightly knotty, tender vein/tendon/ (not sure) under my right arm which includes a stretched feeling down my arm when I straighten it. My skin on my arms has become very dry and crinkly and I also have what looks like a piece of scalded skin on my right thumb base - I have had this for about 7 years but it is slowly getting larger.
