Have Sore Gums And Toothache. Had Molar Extracted. Is It Normal?

Hi so I m pretty sure its my first molar on the bottom right side that i got extracted on Monday Feb. 11th. For the first 2 days I slept and took the painkillers and Ibuprofen that they had prescribed but since then my gum on the outside feels sore and kind of raw. As if my cheek is rubbing against it and the second premolar feels like its shifting downwards a little bit. Sometimes it still feels like i have a massive tooth pain there and also my teeth on the other side of my mouth are starting to hurt a little as well(Also the tooth they extracted was abscessed). Is this all normal? I m 21 and haven t had a tooth removed since I was 5 and it s kind of freaking me out that my mouth hurts this bad still since its been 5 days. Also I was curious on why I m 21(22 in may) and haven t gotten my wisdom teeth in yet.

hi and welcome to HCM,
As you have mentioned the first molar you got extracted was involved with abscess. so after extraction the socket needs to be curretted to remove the pus pocket and all the debris and irritants , disinfection of the socket is done and then the wound is packed with cotton gauze .
any remnants of abscessed tissue in the socket may lead to infection and pain.

Secondly you may have developed a condition of Dry socket due to dislodgement of bloot clot from the extraction wound which leads to delayed healing, pain and discomfort.

you need to get it clinically examined by a dentist to confirm and make a diagnosis.

Pain the other teeth may be due to gingival infection which can be cured by scaling and root planing.
About wisdom teeth , they normally erupt at the age of 18 to 27 years so you can still wait as you will get the eruption signs once they appear.

i would suggest you to get an OPG radiograph done which will show the wisdome tooth appearing as well as infection in other teeth if any , will help in deciding treatment.

hi,
Looks like u have a dry socket.
Teeth that have osteosclerotic alveolar walls because of chronic infection(in your case an abscess) are predisposed to dry sockets. Commonly develop after 3 or 4 days of tooth removal.
Characterized by severe pain and necrotic odor.

Maintain the wound hygiene( warm saline mouth rinse, or an antiseptic mouthwash)
Take the medications the dentist has prescribed, and let nature take its course.

And for your wisdom tooth, u can get an X-ray (OPG) done to observe their position and take measures accordingly.

Wisdom teeth that are painful are often infected. Removing these teeth can alleviate the infection, and subsequently, the pain. Over-the-counter analgesics are common for treatment of wisdom tooth pain. Narcotic drugs, such as codeine, hydrocodine, and oxycodne, are also sometimes prescribed.
The gentle massaging of the cheek and the application of ice to the jaw can also help manage wisdom tooth pain.
Just a small spoonful of normal table salt in a cup of hot water, and then take a sip and tilt your head so that the hot water sits over the sore gum. Wait 10 seconds, spit out, and take another sip.

The pain may be due to many reasons-

pressure applied on the adjacent tooth during tooth extraction.

dry socket arised at the extraction socket area.

pressure exerted by the erupting wisdom tooth.

gum infection due to poor oral hygiene maintenance.

I WOULD suggest you to visit a dentist and get a thorough check up done.

OBTUNDANT dressing using ZOE AND iodoform paste has to be done.
FREQUENT SALINE RINSES MAY BE HELPFUL.

Regarding wisdom tooth eruption ,it takes around the age of 25-26 years,depending on the position and space available for tooth eruption.

Hi,
Thank you for the query.
Removal of first molar which had abscess suggest of the active infection present in such case the socket has to be curettaged to remove the granulation tissue and the pus and then irrigated with betadine , in such procedures usually because of trauma there could be delayed healing leading to dry socket formation and inflammation in the gums .
A course of antibiotics and analgesics along with irrigation of the socket with betadine solution and zincoxide eugenol pack placement should be done for proper healing.
As there was pus , there is posibility of spreading of infection into adjacent teeth supporting area(gums and bone ) which might lead to tilting of the adjacent teeth as the supporting bone might have damaged.
This kind of tilting will lead to occulsal disturbances which in turn lead to pain on the other side of the jaw .
Hope this information helps you.

Wisdom teeth not erupted nothing to worry , get an x-ray done and rule out whether the teeth is present or absent , if present whether it is impacted .
See your dentist to rule out the exact cause for the pain and swelling .

dear friend , thanx for sharing you concern,

don't worry, you have done right thing by getting your abscess molar extracted, now you know that it is healing wound, so definitely you will have little discomfort for a week or so . once the wound heals completely you will have no problem .

you can continue with your same pain killer for another three days twice daily .

do warm saline rinse very slowly without disturbing the clot (healing socket).

i am sure your problem will subside by this.

in case if the pain persist then you need to visit your dentist again else not required .

you have also mentioned about shifting of your premolar so ,just do not worry at all i. it is sensation you are feeling due to extraction of adjacent teeth.

you also have little swelling in that area , which is a part of healing that makes your cheek rub against your wound .

do not worry about wisdom teeth some times it does not erupt in to the oral cavity but till the time it is not causing you any problem , be problem free.

if you want to make sure about your wisdom teeth, get a xray done for exact shape, size and location, also it will help you to understand you whether it will erupt or not.
