If at all you have had unprotecred sexual contact recently, then pregnancy should be ruled out and if at all pregnancy is confirmed this would be a case of failure of contraception.
But before this, you should know that all your symptoms can be caused simply by a hormonal imbalance that can easily be caused by mirena itself. So if pregnancy is ruled out, then the cause is simply mirena itself. Speak to your doctor about the same.
