Hello Dr. Siegel. My Recent CT Myelogram Indicates Multilevel Degenerative

Hello Dr. Siegel. My recent CT Myelogram indicates Multilevel degenerative changes of the lumbar spine including facet arthropathy, and severe intervertebral disc space narrowing at L5-S1. There is also mild right and mild to moderate left neural foraminal stenosis at this level. Degenrative changes of bilateral hips was described on the preceeding scoliosis survey; pain of which is beginning and worsening over the last 2 months. Failed back surgery has also resulted from L5-S1 laminectomy and discectomy 12 years ago. I am an otherwise healthy 50-year-old male, with unpredictable (seemingly inflammatory) nerve pain down both legs, which I thought was Adhesive Arachnoiditis (described as a possibility on my 1st MRI, 7 months postsurgical. However, the recent CT also indicates my lumbar nerve roots are not clumpped. I occasionally have the sensation of water dripping down my legs below the knees, incessant tinnitus, and the nerve pain down both legs is unimaginably horrible beginning just 7 months after the above noted surgery, 12 months ago. All seems hopeless, and I'm wondering if I should even bother seeking neurosurgical help for the 5th time, and/or is there a nationally renowned specialty clinic you might know of?
